Claims Procedures

Once a loss covered by the policy has occurred, immediate written notice must be given to the Company. The written notice should contain information sufficient to identify the named insured. 

The identifying information should at least include the following:-

•         Name of insured

•         Name of policy owner (if different from insured)

•         Policy number

•         Date of loss

•         Whether loss is due to accident or natural causes

•         Whether loss is fatal or non-fatal.

Upon receipt of a notice of claim, we furnish the claimant with a list of documents as are usually required by the Company for filing proofs of loss, and continue guiding the claimant until the claim is concluded.

Documents we require for processing ordinary life death claim notification may be sent online but for all other forms, please print, complete and send to us at the Claims Department on 3rd floor of Pioneer house

•             Claim notification form (if not sent) download

•             Original death certificate

•             Original policy document

•             Attending physician’s statement

•             Identification statement be completed as per instructions on the form

•             Claimant’s statement

•             Original Identity card/passport of both deceased and claimant

•             Original birth certificate and original ID/Passport of the guardian, if designated beneficiary is a minor

•             Police abstract and post-mortem report, if loss is due to an accident

•             Case summary & copies of results of all tests done by the last attending physician

Group Life Claim:

•             Claim notification may be sent online but for all other forms, please print, complete and send to us at the Claims Department on 3rd floor of Pioneer house

•             Claim notification form (if not sent) download

•             Original certificate of insurance

•             Salary slip for the month prior to death

•             Original death certificate

•             Original national identity card of the deceased/copy/surrender letter of identity card

•             Original national identity card of the claimant

•             Attending physician’s statement

•             Identification statement

•             Proof of death form completed by both the employer and the beneficiary

•             Original police abstract and post-mortem report (if loss is accidental)

•             Case summary and copies of results of all tests done by the last attending physician

•             Copy of loan agreement

•             Loan outstanding as at the time of death

•             Letter from the doctor recommending retirement on medical grounds

•             Letter of retirement on medical grounds

3.            Walk in to any of our appointed hospital or medical center and produce your medical card

4.            Fill in the claim form and sign after you have seen the doctor

At Pioneer Assurance Company we are aware that this can be a trying moment for you and your family and it our responsibility to accord financial support as soon as possible.

1.            Call the number on the card (2220814/5) upon happening of death.

2.            Forward death notification letter immediately to the nearest pioneer office.

3.            Fill a claim form available at a pioneer office or download and submit with the burial permit.

4.            Wait for upto 48hrs for a cheque of ksh. 50,000.

5.            The balance of ksh 50,000 will be paid on presentation of original certified copy of death certificate.
